Its active form, calcitriol, regulates calcium balance and is <\/span><span dir=\"ltr\" role=\"presentation\">synthesized in peripheral cells in response to immune signals. Vitamin D, essential <\/span><span dir=\"ltr\" role=\"presentation\">human survival, plays protective and regulatory roles across all body systems. Its mo <\/span><span dir=\"ltr\" role=\"presentation\">active metabolite, calcitriol [1,25(OH)\u2082D], functions through two primary mechanism <\/span><span dir=\"ltr\" role=\"presentation\">intracellular signaling via intracrine and paracrine pathways and genomic actions <\/span><span dir=\"ltr\" role=\"presentation\">mediated by its receptors. For these effects to occur, calcitriol must be synthesized <\/span><span dir=\"ltr\" role=\"presentation\">within peripheral target cells, as its circulating levels are over 900-fold lower than <\/span><span dir=\"ltr\" role=\"presentation\">concentration required to diffuse into them. In contrast, the hormonal form of <\/span><span dir=\"ltr\" role=\"presentation\">calcitriol is synthesized in renal tubular cells and released into circulation. Optimal <\/span><span dir=\"ltr\" role=\"presentation\">serum 25(OH)D levels (40-80 ng\/mL) are essential for preventing infections, <\/span><span dir=\"ltr\" role=\"presentation\">autoimmune diseases, and chronic conditions. However, based on flawed studies an <\/span><span dir=\"ltr\" role=\"presentation\">biases, current guidelines fail to address its broader benefits. Unlike renal tubular cells, whic <\/span><span dir=\"ltr\" role=\"presentation\">maintain continuous calcitriol production in a steady state, peripheral target cells <\/span><span dir=\"ltr\" role=\"presentation\">synthesize calcitriol intermittently in response to chemical signals from cell-surface <\/span><span dir=\"ltr\" role=\"presentation\">immune recognition receptors following antigenic or microbial threats. As a result, <\/span><span dir=\"ltr\" role=\"presentation\">calcitriol synthesized in the kidneys or administered pharmacologically (i.e., calcitrio <\/span><span dir=\"ltr\" role=\"presentation\">or its 1\u03b1-analogs) does not effectively enter peripheral target cells [4]. These cells, <\/span><span dir=\"ltr\" role=\"presentation\">therefore, rely on circulating vitamin D\u2083 and 25(OH)D to meet their functional <\/span><span dir=\"ltr\" role=\"presentation\">requirements. Consequently, physicians should avoid prescribing calcitriol or its <\/span><span dir=\"ltr\" role=\"presentation\">analogs for conditions other than chronic kidney disease [5].<\/span><\/span><span id=\"p28R_mc2\" class=\"markedContent\"><br role=\"presentation\" \/><\/span><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 16px; color: #000000;\"><span id=\"p28R_mc2\" class=\"markedContent\"><span dir=\"ltr\" role=\"presentation\">Different tissues exhibit varying thresholds for adequate vitamin D diffusion fro <\/span><span dir=\"ltr\" role=\"presentation\">circulation.<\/span><\/span><span id=\"p28R_mc3\" class=\"markedContent\"> <span dir=\"ltr\" role=\"presentation\">For example, musculoskeletal tissues function optimally at serum 25(OH <\/span><span dir=\"ltr\" role=\"presentation\">levels of approximately 20 ng\/mL, whereas the cardiovascular and gastrointestinal <\/span><span dir=\"ltr\" role=\"presentation\">systems require 30\u201340 ng\/mL concentrations. For immune system optimization, can <\/span><span dir=\"ltr\" role=\"presentation\">prevention, and longevity, serum 25(OH)D levels exceeding 50 ng\/mL are necessary <\/span><span dir=\"ltr\" role=\"presentation\">3]. The latter include cardiovascular diseases, <\/span><span dir=\"ltr\" role=\"presentation\">diabetes, cancer, respiratory illnesses, chronic kidney disease, immune dysfunction, <\/span><span dir=\"ltr\" role=\"presentation\">autoimmune conditions, neurological disorders such as Alzheimer\u2019s disease, and <\/span><span dir=\"ltr\" role=\"presentation\">complications during pregnancy [6]<\/span><\/span><\/span><\/p>\n<p><strong><span dir=\"ltr\" role=\"presentation\">Vitamin D deficiency (VDD) is widespread globally, even in regions with a bund <\/span><span dir=\"ltr\" role=\"presentation\">sunlight.<\/span><\/strong><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 16px; color: #000000;\"><span dir=\"ltr\" role=\"presentation\">In the United States, approximately 40% of the population has deficient 25(OH)D lev <\/span><span dir=\"ltr\" role=\"presentation\">(&lt;20 ng\/mL), while rates in Central Europe exceed 50% [7]. In tropical regions such <\/span><span dir=\"ltr\" role=\"presentation\">South Asia and the Middle East, VDD prevalence surpasses 60%, primarily due to su <\/span><span dir=\"ltr\" role=\"presentation\">avoidance behaviors and darker skin tones, which reduce vitamin D synthesis. Maintaining 40\u20138 <\/span><span dir=\"ltr\" role=\"presentation\">ng\/mL concentrations is critical for protecting against infections, autoimmune <\/span><span dir=\"ltr\" role=\"presentation\">diseases, and chronic conditions [1, 8]. Consequently, researchers should prioritize well-design prospective, community-based clinical studies. Unfortunately, vitamin D guidelines <span dir=\"ltr\" role=\"presentation\">have relied too heavily on flawed RCTs, leading to inadequate clinical<\/span><\/span><br role=\"presentation\" \/><span style=\"font-size: 16px; color: #000000;\"><span dir=\"ltr\" role=\"presentation\">recommendations.
