color: #000000;\">conditions in your large gut only.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">This last part of your digestive system functions to <\/span><span style=\"font-size: 24px; color: #000000;\">mainly inflammatory and are:<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2022 Ulcerative colitis<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2022 Crohn\u2019s disease<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2022 Spastic colon (Irritable bowel syndrome)- IBS<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2022 Diverticular disease<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2022 Colon Cancer<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Both Ulcerative colitis and Crohn\u2019s diseases, the inner<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">lining of the large gut can undergo a process called<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">\u2018dysplasia\u2019 which is prone to cancer.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Even having Irritable Bowel Syndrome can increase your<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">risk of colon cancer.<\/span><br \/>\n<strong><span style=\"font-size: 24px; color: #000000;\">Let\u2019s now talk about UlLet\u2019s now talk about Ulcerative colitis.<\/span><\/strong><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 24px; color: #000000;\">It is an inflammatory disease which affects the innermost lining of your large intestine.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">It is a disease which debilitates you with life-threatening issues like turning into cancer.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">The inner lining of the large gut is seen red or inflamed with ulcers scattered or localised to one region.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 24px; color: #000000;\">Exact cause of this condition is unknown, although it\u2019s thought to be the result of a problem with the immune system, that is the defence system.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Genetics- Inherited genes are a factor in the development of ulcerative colitis.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">And of course, there are environmental factors, too Radiological examination, that is through barium enema X rays- demonstrates haustral folds in the entire descending colon with small ulcerations.<\/span><\/p>\n<p><span style=\"font-size: 24px; color: #000000;\">There is no specific treatment for ulcerative colitis, only temporising measures to relieve symptoms.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Studies have shown probiotics may improve intestinal mucosa barrier function and immune system function<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">and promote secretion of anti-inflammatory factors,thereby inhibiting the growth of harmful bacteria in the <\/span><span style=\"font-size: 24px; color: #000000;\">intestine.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"color: #000000;\"><span style=\"font-size: 24px;\"><strong>Crohn\u2019s disease<\/strong> is also an inflammatory disease like <\/span><span style=\"font-size: 24px;\">ulcerative colitis, that can affect any part of the <\/span><span style=\"font-size: 24px;\">gastroesophageal tract from the mouth to the anus. <\/span><span style=\"font-size: 24px;\">Symptoms of both ulcerative colitis and Crohn\u2019s disease <\/span><span style=\"font-size: 24px;\">are similar and there is no cure, but only temporizing<\/span><\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">measures.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\"><strong>Diverticular disease<\/strong><\/span><br \/>\n<span style=\"color: #000000;\"><span style=\"font-size: 24px;\">In this condition small bulges called diverticula- like little <\/span><span style=\"font-size: 24px;\">sacks pouch out from the wall of the large gut. They are <\/span><span style=\"font-size: 24px;\">most common in the sigmoid colon- part of the gut <\/span><span style=\"font-size: 24px;\">above the rectum.<\/span><\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 24px; color: #000000;\">The most common cause attributed to the condition i<\/span><span style=\"font-size: 24px; color: #000000;\">dietary- a low in fibre and high in fat diet results in the <\/span><span style=\"font-size: 24px; color: #000000;\">formation of hard stools and consequently the pressure <\/span><span style=\"font-size: 24px; color: #000000;\">within the bowel rises and results in the pouches. <\/span><span style=\"font-size: 24px; color: #000000;\">These sacs remain dormant for a long time and gives no<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">problems.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">The condition is diagnosed through barium meal enema <\/span><span style=\"font-size: 24px; color: #000000;\">x rays and through colonoscopy.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Common symptoms are abdominal distension and pain <\/span><span style=\"font-size: 24px; color: #000000;\">in association with diarrhoea or constipation.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">They can get inflamed and abscesses may form around<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">the diverticula and antibiotics seem to settle <\/span><span style=\"font-size: 24px; color: #000000;\">temporarily. <\/span><span style=\"font-size: 24px; color: #000000;\">Perforation or with failure of medical therapy, segmental <\/span><span style=\"font-size: 24px; color: #000000;\">resection surgery may be indicated.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\"><strong>Cancer of the large bowel<\/strong><\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Risk of Cancer of the large bowel is prevalent among Sri <\/span><span style=\"font-size: 24px; color: #000000;\">Lankan population.Colorectal cancer is the fourth <\/span><span style=\"font-size: 24px; color: #000000;\">leading cancer among men and sixth leading cancer<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">among women in Sri Lanka, according to authors Yasara Samarakoon, Nalika Gunawardena, Aloka <\/span><span style=\"font-size: 24px; color: #000000;\">Pathirana.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">They further said that the high prevalence of being \u2018at <\/span><span style=\"font-size: 24px; color: #000000;\">risk\u2019 of colorectal cancer indicates the necessity of <\/span><span style=\"font-size: 24px; color: #000000;\">screening for colorectal cancer as a programme in Sri <\/span><span style=\"font-size: 24px; color: #000000;\">Lanka. Considering the logistic difficulties in offering <\/span><span style=\"font-size: 24px; color: #000000;\">follow-up diagnostic colonoscopy examinations for all,<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">the study recommends that those found as \u2018high-risk\u2019 be <\/span><span style=\"font-size: 24px; color: #000000;\">eferred for colonoscopy.<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 24px; color: #000000;\">Furthermore, the high incidence occurs under the age of<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">50 years <\/span><span style=\"font-size: 24px; color: #000000;\">Why such a high incidence of colon cancer among Sri <\/span><span style=\"font-size: 24px; color: #000000;\">Lankans, when only a very small amount is linked to <\/span><span style=\"font-size: 24px; color: #000000;\">inherited genes. The most inherited syndromes that <\/span><span style=\"font-size: 24px; color: #000000;\">increase colon cancer risk is the high incidence of polyps <\/span><span style=\"font-size: 24px; color: #000000;\">in the large bowel.\u00a0<\/span><\/p>\n<p style=\"text-align: justify;\"><span style=\"font-size: 24px; color: #000000;\">One reason why the incidence among Sri Lankans is the <\/span><span style=\"font-size: 24px; color: #000000;\">consumption of red meat from cattle seems to be high. <\/span><span style=\"font-size: 24px; color: #000000;\">Cattle red meat is much cheaper that goat meat, and<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">the average person can well afford to purchase cattle <\/span><span style=\"font-size: 24px; color: #000000;\">red meat. In most beef stalls in the main cities like <\/span><span style=\"font-size: 24px; color: #000000;\">Colombo and Kandy, the beef seems to sell fast, and <\/span><span style=\"font-size: 24px; color: #000000;\">you need to sometimes stand in a queue when the <\/span><span style=\"font-size: 24px; color: #000000;\">carcasses are brought with the health approved seal<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">from the health authorities, about 10 o\u2019clock in the <\/span><span style=\"font-size: 24px; color: #000000;\">morning.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">The incidence of consumption of pork is high among the <\/span><span style=\"font-size: 24px; color: #000000;\">population in the coastal regions of Sri Lanka.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Processed meats, like sausages, hot dogs, bacon are <\/span><span style=\"font-size: 24px; color: #000000;\">freely available in the supermarkets today, and they<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">seem to sell fast.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Processed foods that are high in sugar and low in fibre <\/span><span style=\"font-size: 24px; color: #000000;\">and nutrients have been linked to a higher cancer risk in <\/span><span style=\"font-size: 24px; color: #000000;\">the colon.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Researchers have found that a diet that causes blood <\/span><span style=\"font-size: 24px; color: #000000;\">sugar levels to spike is associated with an increased <\/span><span style=\"font-size: 24px; color: #000000;\">incidence and risk of large bowel cancer. <\/span><span style=\"font-size: 24px; color: #000000;\">Diets that include lots of vegetables, fruits and whole <\/span><span style=\"font-size: 24px; color: #000000;\">grain have been linked with a decreased risk of colon<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">cancer.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">In Sri Lankan markets, leafy veggies are cheap, but not <\/span><span style=\"font-size: 24px; color: #000000;\">all veggies like beetroot, tomatoes, potatoes and many <\/span><span style=\"font-size: 24px; color: #000000;\">others are out of the reach of the average wage earner.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Obesity also seem to increase the risk of large bowel <\/span><span style=\"font-size: 24px; color: #000000;\">cancer. Excess body weight contributes to insulin <\/span><span style=\"font-size: 24px; color: #000000;\">resistance and eating more carbs and foods with added <\/span><span style=\"font-size: 24px; color: #000000;\">sugars seem to be the cause for obesity, in such <\/span><span style=\"font-size: 24px; color: #000000;\">situations.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">There are certain superfoods that fight and prevents <\/span><span style=\"font-size: 24px; color: #000000;\">cancer such as most veggies containing cancer-fighting <\/span><span style=\"font-size: 24px; color: #000000;\">antioxidants and phytochemicals. <\/span><span style=\"font-size: 24px; color: #000000;\">For example, cruciferous vegetables, including broccoli, <\/span><span style=\"font-size: 24px; color: #000000;\">cauliflower and cabbage, contain sulforaphane, a <\/span><span style=\"font-size: 24px; color: #000000;\">substance that has been shown to reduce tumour size in <\/span><span style=\"font-size: 24px; color: #000000;\">mice by more than 50%<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Vegetarians including vegans who consume plant-based <\/span><span style=\"font-size: 24px; color: #000000;\">foods have a reduced risk of developing or dying from <\/span><span style=\"font-size: 24px; color: #000000;\">colon cancer.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Ketogenic diet lowers blood sugar and insulin levels <\/span><span style=\"font-size: 24px; color: #000000;\">seem to reduce the risk of colon cancer.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Bottomline<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">All inflammatory diseases and cancer of the large bowel <\/span><span style=\"font-size: 24px; color: #000000;\">are incurable conditions. Cancer can be cured if <\/span><span style=\"font-size: 24px; color: #000000;\">detected early. <\/span><span style=\"font-size: 24px; color: #000000;\">It is reasonable to say that you should see your doctor<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">for an annual screening for cancer, if there is such risk <\/span><span style=\"font-size: 24px; color: #000000;\">and a family history.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Chronic inflammatory diseases must be diagnosed early <\/span><span style=\"font-size: 24px; color: #000000;\">for better palliative care and longevity.<\/span><br \/>\n<span style=\"font-size: 24px; color: #000000;\">Hope this talk was useful.
